5 Freeway Cleanup - RSVP Form

In cooperation with CalTrans, the shop has sponsored two miles of the southbound 5 freeway in the Griffith Park & Los Feliz area. For permission to access the freeway, we need to give CalTrans a few days notice. For that reason I need RSVPs from all participants.
